Leyte Normal University invites all eligible students to submit their applications for graduation. Please complete your requirements within the designated period to be included in this year’s graduating class.

• Graduating Students with COMPLETE requirements

Requirements

• Duly filled-out Application for Graduation Form. Click this link to download.
• Complete grades (including PATHFIT, NSTP, etc.)
• All original entrance documents:
  • SHS Report Card & Permanent Record (Form 137)
  • Transfer credentials (for Transferees)
  • PSA/NSO Birth Certificate
  • Certificate of Good Moral Character
• Passport-size ID Picture (with white background)

Schedule of Submission

• May 4-8, 11-15, 2026 at 8:00 AM to 5:00 PM

Where to Submit?

• LNU Registrar’s Office, 1/F Administration Building

Note:

• Lacking documents may be submitted together with the application form.
• Applications with incomplete grades and requirements WILL NOT be processed.
